In order for me to be most useful and for you to maximize your counseling experience, please be prepared to share the following information during your first session:
- Reason you are seeking counseling services
- Goals you would like to achieve from counseling (we will talk more about treatment goals once counseling has started)
- Past/present medical and mental health history (including psychotherapy, psychiatric history, substance use, medications, hospitalizations)
- Family medical and mental health history
- Involvement in any current or pending criminal or civil litigation, lawsuit(s), divorce or custody disputes
